Request policy

As part of the Partners in Pediatrics’ medical record request policy, a written request from the parent or guardian of the patient is required. The request can be faxed, mailed or dropped off at any of our five clinic locations. Processing requests for copies can take up to five working days. Release of Information will release copies of medical records only.

NOTE: Children’s Minnesota will not make copies of records from other medical centers. These records must be requested from the originating facility.
